One of these type apps that I have found useful, and avoids Italian tomatoes, is Tadama Workflow.
I like the P------o idea too. With this one you can set the break for just seconds. The recommendation that floats around is 20 minutes of screen, 20 seconds of looking at something else 20 feet away.
